What the Job Involves
This role involves executing safe and timely discharge plans, supporting vulnerable individuals to integrate back into their communities. You will work closely with health colleagues to ensure a seamless transfer from hospital care to community support, developing person-centered plans that reflect the goals and outcomes the service users wish to achieve. Responsibilities include managing caseloads, conducting assessments, facilitating hospital discharges, safeguarding tasks, and potentially handling duty work and on-site assessments at Wexham Park Hospital.
