Who You Are

We are looking for a highly skilled and confident social worker to join our dynamic hospital discharge team. This position requires a professional who can manage complex discharges and transition individuals from the D2A pathway into long-term care within tight timeframes. You will be representing the Adult Social Care at Slough Borough Council, and work within the vibrant and fast paced social work team at Wexham park hospital, focusing on supporting the most vulnerable people in Slough by engaging with partners.

What the Job Involves

Your primary responsibility will be to lead on complex discharges from hospital settings, coordinating timely and appropriate long-term care solutions. You are expected to navigate assessments, funding pathways, and family dynamics with professionalism and empathy, ensuring compliance with legal and policy requirements while maintaining a person-centred approach. The role involves safeguarding tasks, especially for any abuse that occurred within the hospital, and necessitates regular travel within the local area to complete assessments and attend CHC/SG meetings. Working in a multi-disciplinary team, you'll ensure safe discharges with a focus on holistic assessments.

Skills

  • 5+ years in adult social care, ideally with hospital discharge or D2A experience
  • Self-starter with the ability to progress cases independently
  • Strong understanding of the Care Act, Continuing Healthcare (CHC), and safeguarding
  • Excellent decision-making and risk management skills
  • Effective multi-agency working and collaborative skills
  • Calm and confident communicator in fast-paced environments
  • Experience with LAS system is advantageous
