What the Job Involves
Your role involves undertaking needs assessments to identify care and support requirements, collaborating with safeguarding colleagues, and designing care packages adhering to eligibility criteria. You will engage with individuals, families, and communities, helping them make informed decisions. Tasks include maintaining records, writing reports, and possibly providing court evidence. You are expected to work within policies and act according to Lincolnshire County Council Adult Care procedures.
